Unit Head Affidavit for ITA Exam

Per the ITA Policy, the employing unit head can submit this affidavit stating that the unit head has firsthand knowledge that the individual possess sufficient English skills to warrant access to ITA testing despite not meeting the iBT or IELTS score requirements or if they took the PTE Academic exam.

Please complete this form for each person within your program who requires an affidavit for ITA testing. 

If approved, the student should register for the ITA Exam (if s/he has not already done so).
